Enrollment Process – Woodbury

Begin your enrollment by completing our Enrollment Agreement. There are two options. You may use our online form or you may download and print a hard copy and return it to our front desk. 

To complete your registration at Peace of Mind Woodbury, you will also need to fill out our Enrollment Registration package. A link to the online registration is below. A paper version is available upon request. You will be asked for information regarding immunizations, insurance, emergency contacts etc. Please have this information easily available before you start as you will not be able to save your enrollment form and come back to it later.

If there are special dietary concerns, schedule and complete a meeting with the center’s head cook. Our administration team will make certain you receive any other pertinent forms such as school calendar, class directory, etc.
